“You can apply to register whether you live in or out of the practice catchment area.”
If you live IN the practice area.
New patients who permanently reside within the current practice boundary who wish to register should come in to collect a registration pack from reception or you can download it below.
You do not need to provide proof of address but we ask that you bring a form of ID to reception for your first appointment. This is to ensure accuracy of your registration only and is not compulsory.
It can take up to 5 working days to be on our system once all required documents have been received in the surgery.
If you live OUT of the practice area
GP practices now have the option to register patients who live outside the practice area but without any obligation to provide home visits.
Out of area registration (with or without home visits) is voluntary for GP practices meaning patients may be refused because they live out of area.
Your application will be considered by the practice and will only register you without home visits if it is clinically appropriate and practical in your individual case.
You can register online by filling out the GMS1 and the New Patient Health Questionnaire Form. You must also,download and sign “Consent for out of catchment area home visits” form and send it (either take a photo or scan document) and send as an attachment to email@example.com
Alternatively please visit the practice and advise one of our reception team that you would like to register as an “out of area patient” and they will provide you with the necessary paperwork to make an application.
Please note that if accepted, but your health needs change, we may review your registration to see if it would be more appropriate for you to be registered with a GP practice closer to your home.
Please note we do not accept new registrations in the surgery after 12pm on Friday’s.

Your Named GP
The surgery provides all patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them.
We need to divide our patients evenly between all of our doctors, so they can manage their workload. Therefore to do this fairly and impartially, we have selected patients by surname to match to a doctor.
We know that some patients will wish their named GP to be a particular doctor. Where a patient expresses a preference as to which doctor they have been assigned, the practice will make reasonable efforts to accommodate this request, but we would like to reassure patients that this system does not prevent you from seeing any GP in the practice as you currently do. Your named GP will not be available at all times.
You do not need to take any further action and will be informed of your named GP in due course. In the meantime, if you wish to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery.
